GENERAL INFORMATION AND STATEMENT OF POLICY

Registration Policy: A completed application and the required payment is due at the time of registration. (Please Type Your Initials In the Box.) *

Personal Equipment: All personal equipment is the responsibility of the equipment owner/renter and Diventures Atlanta is not liable for the loss or damage of personal equipment (ie. laptops, phones,etc.) *

It is expressly understood that Diventures Atlanta, its agents and employees, cannot verify either the ability or suitability to pass this class of each participant and therefore cannot guarantee that the participant will pass the class by the lifeguarding instructor. If the proficiency of the student is not acceptable, refunds are not available. (Please Read Carefully and Type Your Initials in the Box.) *

It is expressly understood that all course fees are non-refundable.

If you need to reschedule your class and you provide advanced notice of at least 72 hours (3 days) prior to the date of your class, we will happily reschedule your class at no additional cost.

For reschedules with less than 72 hours (3 days) notice of the date of your class, or if you no-call/no-show, a fee of $50 will be incurred to reschedule to a new date. (Please Read Carefully and Type Your Initials in the Box.) *

Certain medical conditions may not be conducive to lifeguarding. I represent that I have sought the advice of a physician and said physician has advised me that there is no medical reason preventing me from lifeguarding. *

Photo Release: SeaVentures has the right to photograph students for online and in-store publications and flyers. If photos are sent to us by students, SeaVentures has the right to use those photographs for online and in-store publications and flyers. *
