Thanksgiving & Winter Break H2O Camp

Children 5 years old and up can spend the week with SeaVentures during Thanksgiving & Winter Break swimming, playing games, making themed crafts, watching movies, and more! After a swim evaluation to gauge their level, they receive 2 swim lessons per day to fine tune their skills and get them comfortable in the water.

Children must bring a towel, goggles, and a change of clothes.

Picking Up Your Child

We have a daily sign in sheet to know who is planning on picking up the child. If someone not on this form is going to pick up your child on a specific day, it needs to be written on the daily sign in sheet. Anyone not listed on the emergency form or on the daily sign in sheet will not be allowed to pick up your child from camp. Pick up time is 5:00-5:30pm (Set to Seaventures Pool clock). Any time after 5:30pm will be charged $1 per minute. Please initial at the end of this statement that you acknowledge the procedure of picking up your child.

Terms and Conditions

SWIMMING INFO
We will do a brief swim test to make sure all children are safe and able to be in the pool without personal floatation devices. Please initial at the end of this statement to acknowledge you understand that if your child is unable to swim the proper distance in the pool safely, he/she will be required to wear a flotation jacket during all water activities until the swim test is passed.

GENERAL SWIM TERMS AND CONDITIONS

Class Minimums: A minimum number of students are required in order for SeaVentures Aquatic Center to hold a class. In the event that SeaVentures Aquatic Center cancels a class due to low registration, either the class may be cancelled/moved to another time or the class will be 20 minutes in length. We will work with you to find an alternative class in the current or upcoming session. If no appropriate alternative is available, SeaVentures Aquatic Center will issue a credit for the entire registration amount.

Class Check-in: Upon arrival, your child's temperature will be checked at the door. Parent or other listed party must sign children in and out.

Refund Policy: NO REFUNDS. We hire and schedule counselors based upon registration numbers.

Make-Up Policy: There are no refunds or session transfers. A makeup or store credit can only be given in the event of an illness with a doctor’s note. Makeups are subject to class availability and are not guaranteed. We do not offer makeups for any scheduled makeups that are missed. We do not offer credits or refunds for missed classes. Makeups expire when you are no longer enrolled. We do not offer makeups for private lessons.

Schedule Change: Schedule changes are subject to class/session availability. You may change your child's class schedule for the remainder of a session. You may do this once at no charge. Additional changes will require a $10 fee for processing. If an instructor advises a change in the middle of the session, there will be no charge.

Illness Policy: If your child is absent from class due to illness, a store credit will be issued if a note from the doctor is presented within 7 days of the student’s absence. The note must have the dates that the child was not allowed to participate in class. No credit is issued without a note from the doctor.

Credit Policy: Credits are issued only for an illness with a doctor’s note or pool closures. Credits are not issued for conflicts with other activities or vacations. However, you can be issued a store credit. Credits are placed on the customer’s account for future use. Credits are not redeemable for cash. If a refund is returned back onto a credit-card, a 3% processing fee is required.

Inclement Weather: All SeaVentures Aquatic Center lessons are held at our indoor facilities. Therefore, cancelations due to weather are infrequent. If we close the facility for inclement weather, we will issue a credit.

Pool Closings: SeaVentures closes its pools for the following reasons: (1) When a child vomits or has a bowel movement in the pool; (2) Inclement weather; (3) If conditions in the pool make it unsafe for students to be in the pool. When the pool is closed for any of the above reasons, a credit will be issued.

Pool Closing Fee: If your child vomits or has a bowel movement in the pool thereby causing the pool to be closed, you will be assessed a $250 contamination and cleaning fee. Your child may not participate until this fee is paid.

Swim Instructors and Camp Counselors: We do not guarantee specific teacher/counselor assignment. Also, if you need to speak to an instructor, it is best to wait until the class is over OR provide the store staff with your contact information to be contacted after the instructor is out of the water.

Storing Credit/Debit Card on File: If you choose to save your credit-card information on file, SeaVentures has the right to withdraw funds from the card on file upon your request.

Georgia Department of Early Care and Learning Exemption Acknowledgment

I understand that I am being informed in writing and by signing this acknowledgement that this facility, SeaVentures, Inc., has filed for/received exemption with Georgia Department of Early Care and Learning. SeaVentures DOES carry liability insurance though Witherspoon & Associates- 3000 Meridian Blvd. #100, Franklin, TN.

Continuing Waiver & Release of Liability, Assumption of Risk & Indemnity, and Emergency Care Permission

PLEASE READ CAREFULLY BEFORE SIGNING BECAUSE THIS IS A CONTINUING RELEASE OF LIABILITY AND WAIVER OF CERTAIN LEGAL RIGHTS. IT ALSO GRANTS CONTINUING PERMISSION FOR EMERGENCY CARE.

In consideration of permitting me, (participant), to enroll in a swim or snorkeling instructional course and/or participate in Swimming, Snorkeling, Swim Parties, Physical Activities, and Related Operations hereafter known as (Water Activities) conducted by any staff member(s) of Diventures or Diventures Atlanta, LLC, in the city of Alpharetta, State of Georgia, beginning on today’s date, I agree for myself, my personal representatives, heirs and next of kin:

I hereby acknowledge that Water Activities are potentially dangerous activities and involve the inherent risk of serious injury (including paralysis), death and/or property damage both in and under the water as well as on the pool deck itself.

I hereby release, waive, discharge and agree not to sue Diventures Atlanta, LLC, their facilities, staff or any of its officers, instructors, agents or employees (the Releases) from all liability to myself, my minor child(ren), my personal representatives, signs, heirs and next of kin for any and all loss or damage and any claim or demands therefore on account of injury to my person or property or resulting in my death, now and forever, arising out of or related to participation and/or instruction in said course, activities or any other related Water Activities that may occur.

I hereby assume full responsibility for any risk of bodily injury, death or property damage, now and forever, arising out of or related to participation and/or instruction in said course, activities or any other swimming/snorkeling operations.

I hereby acknowledge that this Waiver and Release of Liability is intended to be as broad and inclusive as permitted by the laws of the State in which the activities are conducted, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

I hereby assume full responsibility for determining the need for and providing an interpreter for a language other than English within the facility.

I acknowledge that it is my responsibility to provide for my own and/or my child(ren)’s own accident and health coverage while participating in swim activities.

In the event I cannot be reached and/or am incapacitated or otherwise able to give consent, I give permission for emergency medical, surgical and hospital treatment and procedures to be performed by a licensed physician or hospital, when deemed immediately necessary to safeguard my/my child(ren)’s health. I relieve Releases of any and all responsibility for action(s) taken by the doctor(s), hospitals, or other medical care providers in the treatment and attendance of me or my child.

I agree that this waiver, release of liability, assumption of the risk, and consent for emergency medical, surgical and hospital treatment shall be continuing and effective for all Water Activities conducted by or on behalf of the above named Releases for a period of time beginning with the execution of this document and terminating at 11:59 P.M., CST, on the 365th day after the date on which this document was signed.

Photo Release

I hereby authorize Diventures to publish photographs taken of me and/or the undersigned minor children, and/or names, for use in Diventures’ marketing material, social media and website. I release Diventures from any expectation of confidentiality for the undersigned minor children and myself. I attest that I am the parent or legal guardian of the children listed below and that I have the authority to authorize Diventures to use their photographs and names.

I acknowledge that this photo participation is voluntary, and neither the minor children nor I will receive financial compensation. I further agree that our participation in any marketing piece produced by Diventures confers no rights of ownership whatsoever. I release Diventures, its contractors and its employees from liability for any claims by me or any third party in connection with my participation or the participation of the undersigned minor children.
