REQUIRED DOCUMENTS AND INFORMATION
All of the following documents and information are required in order to complete your admission.
- Results of a TB test (or indicate that you are waiting on results)
- Proof of HIV test (we don't need the results, just the proof of testing)
- If female, pregnancy test results
- If pregnant, documentation for your maternity (you may not be more than 15 weeks pregnant at time of intake)
- Copy of Driver's License or Photo ID
- If you have pending legal obligations:
- Court dates and locations
- Attorney phone and fax numbers
- Probation/Parole Officer phone and fax numbers
- Relevant legal documentation
