Prior to uploading your proof of annual flu vaccination, please make sure the documentation includes the following information:

Your full name

Type of vaccine administered

Date of vaccine administration

Clear & legible copy or photo showing all four corners of the document

Vaccine must be from current 2024-2025 Flu Season
We cannot accept badge stickers as proof of annual flu. We recommend using the PassportUSA Flu Form if possible. A member of the Credentialing department will review your submission. If your documentation is not accepted, your Credentialing Support Associate or IES will reach out to you if necessary.
