This declaration form should be used by undergraduate bachelor's students enrolled in the

School of Professional Advancement (SoPA) to declare a major, minor, or professional certificate.

You should submit one form per program that you would like to declare-

you CANNOT declare more than one program on a form.

Post-baccalaureate, professional certificate, and graduate students should contact their

SoPA advisor to change program major/track.

Newcomb-Tulane students should contact their NTC academic advisor for

the correct major/minor declaration form.
