Registration Fees and Cancellation Policy

Ohio-based Company - $100 per class (Waived upon completion of training.)
Registration Fees for Ohio-based companies will be waived when the registrant attends the training. If registrant does not attend and does not cancel a minimum of 7 business days prior to the first date of class, the registration fee will be charged. You may send a substitute in place of the original registrant at no additional charge.

Company Outside of Ohio - $300 per class

A refund less a $25 administrative fee will be granted for cancellations received a minimum of 7 business days prior to the first date of the class. You may send a substitute in place of the original registrant at no additional charge.

I agree that if I do not cancel a minimum of 7 business days prior to the first day of class, I will forfeit a refund or as an Ohio-based company, I will be responsible for the $100 registration fee unless I send a substitute employee.

Course Records
The completion records for OPGA's certified and non-certified courses are managed through PERC's Learning Center. Additionally, testing for certified courses will be conducted online during the class training. OPGA will need to register each student to take the online exam. PERC uses the last 4 digits of an employee/student's social security number as an account identifier. If a student does not have an account already set up with PERC, OPGA will create one for them.
