Hello, Pennsylvania Transition Stakeholders!

Our team, the “Pennsylvania Community on Transition,” created a survey to discover how youth across Pennsylvania set goals for the future and plan for adulthood. We also want to find ways to better help youth and families meet these goals, which includes supporting professionals to get the resources they need. 

We need and value YOUR input and opinions. It will take about 10 minutes to complete this survey. Your responses will remain anonymous and will not be linked to you, your family, your school district, or your agency. Thank you very much for your help.

A youth version of this survey will be available Fall 2021. If you are a parent completing this survey, you will be asked if you would like your youth to participate in the youth version.

*The Pennsylvania Community on Transition is a state leadership team consisting of representatives from: The State Departments of Education, Health, Labor and Industry, and Human Services; Various Service Agencies, Parent and Youth Organizations, Advocates, Higher Education, and Employers. This team works together to support youth with disabilities and their families as they move from high school to post-school activities.
